Abstract:
Post-partum hemorrhage (PPH) remains a leading cause of maternal morbidity and mortality, affecting 3–5% of deliveries in the United States. Current transfusion protocols often treat PPH as either trauma or non-trauma, despite pregnancy’s unique hemostatic physiology. This talk reviews the hematologic changes during pregnancy, current ACOG and FIGO guidelines, and emerging evidence on massive hemorrhage protocols tailored for obstetric patients. We will explore the role of early fibrinogen replacement, goal-directed therapy using viscoelastic testing, and institutional considerations for developing PPH-specific protocols. Attendees will gain insight into optimizing transfusion strategies to improve outcomes in this critical setting.
Objectives:
- Describe the basic physiologic changes in hemostasis and thrombosis during pregnancy.
- Discuss the current recommendations for transfusion support for post-partum hemorrhage.
- Evaluate whether post-partum hemorrhage requires its own massive hemorrhage protocol at your institution.
